As skin cancer rates continue to soar within the United States, many leading experts consider skin cancer a modern epidemic. As is common with various other cancers, early detection provides the most favorable conditions for recovery. Disease progression, along with other factors, often dictates that treatment options may be available to patients, and dictates potential mortality rates for each type of skin cancer. Squamous Cell Carcinoma, specifically, is the second most common type of skin cancer, accounting for roughly 250,000 newly diagnosed cases each year. Most common in older adults with fair complexions, and individuals who have suffered vast sunburns, Squamous Cell Carcinomas may be treated aggressively, dependent upon the disease progression at diagnosis. If diagnosed within its early stages, this type of skin cancer usually remains located centrally, and does not spread to other parts of the body. If left undetected, however, this type of skin cancer can spread to the lymph nodes, and other organs, and can become lethal. In fact, Squamous Cell Carcinoma accounts for roughly 2,500 deaths within the United States annually.\

Dr. Ioannides, along with colleague Dr. Anna Nichols, a dermatologist, and Assistant Professor at the Dr. Phillip Frost Department of Dermatology and Cutaneous Surgery at the University of Miami Comprehensive Cancer Center, published the findings of their groundbreaking HPV vaccine study in 2018. The subject of their study, a 97 year old patient diagnosed with Squamous Cell Carcinoma, was provided with the Gardasil 9 injection. Post injection, the patient’s tumors disappeared. Notably, the patient has not relapsed, and most recently, celebrated her 100th birthday, earning celebratory recognition for the success of her treatment.
Initially, the publication received great recognition within the medical dermatology world, with peers showcasing great interest in the topic of unique cross-utilization of different medications to treat “off label” conditions. The industry renowned JAMA Dermatology website bestowed the piece the honor of naming the study one of the “Most Talked About” publications of the year. One of only ten publications selected for this title, the buzz surrounding the successful utilization of the HPV vaccination for the treatment of skin cancer remained at the forefront of interest within the industry.
Within the published piece, Nichols relayed the previous success in slowing down skin cancer growth for two patients who previously received the Gardasil 9 vaccination. Thus, at the suggestion of fellow faculty member Dr. Ioannides, the team moved forward with injecting the vaccination directly into the tumors of the patient. While off-label treatments have not yet been approved by the Food and Drug Administration for that particular type of cancer, and Gardasil 9, the promising preemptive evidence suggested a potentially hopeful outcome. Though Dr. Ioannides, and Dr. Nichols retained realistic expectations, they were hopeful about the possibilities that existed within the parameters of what could be considered successful within this particular case.
Dr. Ioannides stated,“I think we had a really reasonable expectation and good data that this was actually going to, at the very least, do no harm to this patient and possibly provide some benefit. To have this type of result in such an advanced patient, I think, was beyond all our expectations.” Taking the patient’s health and status into great consideration, all parties involved made reasonable deductions related to the risk versus outcome of this innovative treatment approach. With unprecedented results, this innovative treatment provides a uniquely hopeful approach that can be repeated in the future, and developed into a functioning, and touted, method of treatment.
The success of this trial could have potentially life changing results for individuals who currently would not have many treatment options to choose from, providing hopeful successful results, or the prospect of successful results, in cases where treatment options are slim. In the particular case of the patient chronicled within the published study, the patient was not a candidate for radiotherapy, nor for the surgical removal of the cancerous tumors. Based on the location, size, and extensive nature of the tumors on her right leg, the patient had very few viable treatment options available. Had it not been for this groundbreaking and limit pushing option, the patient’s prognosis would not have been positive.
In this particular case, the patient received an initial injection locally into her arm, with subsequent injections directly into the tumor sites. Within months of injection, the patient’s tumors began to noticeably shrink, disappearing altogether after roughly one year. Remarkably, the results of these injections showcased a full recovery. “Among other things, the before-and-after pictures of our patient are unbelievable,” said Dr. Nichols. “You don’t need statistical analysis to appreciate the improvement. First, you see tumors all over her leg, and then zero. Not a 20 percent reduction but a 100 percent reduction.” In the success of this particular case, a patient who previously would not have been a candidate for other traditional methods of treatment was able to garner the most positive end result, with the complete eradication of cancer cells.
With the extensive success of this initial trial, the Skin Cancer Foundation has awarded a $25,000 grant specifically dedicated toward the continued research related to utilizing the HPV vaccine for the treatment of skin cancer. To date, the market availability of the HPV vaccine has been revolutionary in preventing new cases of the HPV virus, and in the treatment of cancers associated with the virus, including cervical, genital, and oral cancer. The vaccine, however, has not yet been extensively tested in regard to the efficacy of treatment for already existing tumors, and varying types of cancers. However, with the initial successes seen thus far, further medical examination is obviously warranted. With proper funds allocated to the development of this breakthrough treatment option, Dr. Nichols aims to initiate further clinical trials, utilizing the HPV vaccination for the treatment of skin cancers.
